REGISTRATION FORM (signature required to participate in classes)
The "Belly dance Basics with Aepril Schaile" class will be held at The Oracle Chamber,
124 Derby St, Salem MA.
50% deposit required for classes. This will guarantee your spot. Balance due at the
beginning of the first class unless other arrangements have been made. All payments
are nonrefundable after first class. A $25.00 fee will be assessed for any returned

I will need at least 8 people registered for each class by 5/19/08 to make it happen! If
not, two classes may be merged (and appropriate adjustments made to accommodate
different skill levels), or class may be cancelled and deposit refunded.

below, or you can register using paypal (if available) on my website,

with your check, or return it to me in person before class begins

Instructors and facilities are not liable for personal injuries or loss of, or damage to
personal property. Since this is a physical activity, injuries may occur. Each student
may decline to participate in any activity that they feel may be harmful; and is also
responsible to inform the instructor of any physical limitations that may prevent full
participation. A doctor’s written approval is required in order to resume classes after
extended illness, physical injury or accident.
